Request Immunization Records

If you're registering your child for school, you don't need to request an immunization to provide to the school. Report your child's vaccinations using Immunization Connect (ICON).

How to access immunization records using Immunization Connect (ICON)

Access your vaccination record or report vaccines to Public Health using Immunization Connect (ICON). You can view and print immunization records for you or your child. Children 16 years of age and older must request their own records due to privacy laws.

To view or print your vaccination record, you will need to create an account and set a PIN. To do this, you will need one of the following:

  • Ontario Health Card Number, or
  • An Ontario Immunization Identification. Call 905-688-8248 or 1-888-505-6074 ext. 7425 if you need to get one

If you get an error message when trying to log into Immunization Connect, call 905-688-8248 or 1-888-505-6074 ext. 7425.

Common reasons for an error message include:

  • There is no health card number on file for you
  • The postal code is incorrect / outdated. Immunization Connect verifies your identity based on the postal code your health card is attached to.
  • If there is a typo on one of the fields of your vaccination record

If you're unable to get an immunization record using Immunization Connect

If you live or attend school in Niagara, you can also request an immunization record for yourself or your children (under the age of 16). Records make take up to 14 days to be received. Contact the Vaccine Team.

Record availability

It’s important to know that immunization records are not provided to Public Health from doctors, child care facilities or schools.

This means that Public Health may not have an immunization record for you, particularly if you were born before 1980, unless you attended one of our clinics.

Public Health may not have an immunization record for you or your child if you:

  • Have not reported vaccinations to Niagara Region Public Health
  • Just moved to Ontario
  • Received your vaccinations from a health care provider or pharmacist. Most health care providers don't report vaccinations to Niagara Region Public Health.
