Assisted Living

Assisted living provides services in a seniors apartment or congregate living setting on a scheduled basis. Services are non-clinical personal supports that help the individual to remain living independently in an apartment setting.

The program doesn't have registered nursing or other clinical staff on site. Any required nursing or professional care needs can be arranged through Home and Community Care Support Services.

Eligibility

Eligibility depends on the specific needs of the client as assessed by Home and Community Care Support Services.

Prospective clients cannot be on a wait list for long-term care, or need immediate or 24 hour availability of nursing care or other professional medical services.

Services

Assisted living services are for adults who can live independently, but need support with key tasks and activities.

Service plan

The tenant is assessed to identify need and the supports available within the service. Once identified, a service plan is developed and an annual assisted agreement is signed. The agreement outlines the scope of assisted living services, and responsibilities of the provider and the tenant.

The service delivery model requires a level of self-sufficiency where only minor help is needed to be successful when assisted with:

  • Medication prompts
  • Bathing
  • Dressing
  • Laundry
  • Light housekeeping
  • Attendant support

Other services

Other services can be accessed by tenants on a scheduled basis with outside contracted providers. Additional fees are the responsibility of the tenant and include:

  • Foot care
  • Mobility equipment servicing and repairs
  • Transportation
