Screening forms for school
We provide dental screenings annually to elementary school students in participating grades in Niagara.
- I DO NOT want my child to take part in the 2023-2024 school dental screening
- My child is not in one of the participating grades, but I DO want my child to take part in the 2023-2024 school dental screening
- My child is in junior or senior kindergarten and I would like to provide consent for fluoride varnish application
- My child's school will be visited by the Mobile Dental Clinic and I would like to provide consent for dental services
