Niagara Emergency Medical Services provides a wide range of information through its teams in operations, operational support, communications centre, and program development and service quality.
Types of records
Emergency Medical Services provides:
- Ambulance call reports
- Dispatch summary reports
- Audio recordings of incoming 911 call(s) to Niagara Emergency Medical Services
To request other types of information, submit a Freedom of Information request.
How to request records
Allow 30 days for processing from the time the form and payment have been received.

Patient / family / substitute decision maker requests
Before starting your records request, make sure you have:
- Government photo identification
- If you're not the patient, proof of legal authority for substitute decision making, such as power of attorney or will
Complete the Authorization to Disclose Personal Information Form.
Once the record(s) are found, we will invoice you our fee for processing your request.
To submit this records request by mail, email Niagara Emergency Medical Services for a paper application.

Legal representative requests
To submit a request, mail or email nemsrecords@niagararegion.ca the request on company letterhead. You must include:
- Patient's name
- Patient's date of birth
- Date, time and location of the incident
- The hospital the patient was transported to, if applicable
- Signed patient authorization form
Payment
Mail a cheque for the requested amount made out to "The Municipality of Niagara Region" to our accounts receivable department at 1815 Sir Isaac Brock Way, Thorold, ON, L2V 4T7.
A copy of the requested documents will be sent to the requestors email address.
For disclosure requests related to the personal health information of deceased patients, consent must be received from the estate trustee or the individual who has assumed responsibility for the administration of deceased's estate, if the estate doesn't have a trustee.
For these requests, proof of the requestor's authority to consent to the disclosure of the records must be submitted. For example, copy of the last will and testament or certificate of appointment of estate trustee without a will.

Personal health information of deceased patients
For disclosure requests pertaining to the personal health information of deceased patients, consent must be received from the estate trustee or the individual who has assumed responsibility for the administration of the deceased's estate, if the estate doesn't have a trustee.
For these requests, proof of the requestor's authority to consent to the disclosure of the records must be submitted. For example, copy of the last will and testament or Certificate of Appointment of Estate Trustee without a will.
