COVID-19 - As Ontario reopens, learn about public health and workplace safety measures. Learn about the COVID-19 vaccination and service disruptions.

Request Vaccination Records

All records will be mailed due to the temporary closure of all Public Health offices. If you're registering your child for school, report your child's vaccinations online to Public Health. You don't need to request an immunization record to provide to the school.

If you received a COVID-19 vaccine, it won't appear on this record. Get a copy of your Ontario COVID-19 vaccination record using the online Provincial booking system. After you log in with your health card number, you will receive an option to book an appointment or get a copy of your COVID-19 vaccination receipt.

You can request immunization records for yourself or children (under the age of 16) free of charge. Children over the age of 16 must request their own records due to privacy laws.

Records may take up to 14 days to be received in the mail.

  • Just moved to Ontario

  • Were born before 1980 (unless you attended one of our clinics)
