Request Immunization Records and Other Related Documents
If you received a COVID-19 vaccine, it won't appear on this record. To get your COVID-19 vaccine receipt, visit the Ontario COVID-19 vaccination portal.
If you're registering your child for school, report your child's vaccinations online to Public Health. You don't need to request an immunization record to provide to the school.
If you live or attend school in Niagara, you can request immunization records for yourself or your children (under the age of 16) free of charge. Otherwise, call your local public health unit. Children 16 years of age and older must request their own records due to privacy laws.
Records may take up to 14 days to be received.
Record availability
Immunization records are not automatically provided to us. We may not have a immunization record if you:
- Have not reported vaccinations to Niagara Region Public Health
- Just moved to Ontario
- Received your vaccinations from a health care provider or pharmacist (most health care providers don't report vaccinations to Niagara Region Public Health)
- Were born before 1980 (unless you attended one of our clinics)

Answer the following questions based on whether the request is for yourself OR your child. Children 16 years of age and older must request their own records due to privacy laws.
