All records will be mailed due to the temporary closure of all Public Health offices. If you're registering your child for school, report your child's vaccinations online to Public Health. You don't need to request an immunization record to provide to the school.
You can request immunization records for yourself or children (under the age of 16) free of charge. Children over the age of 16 must request their own records due to privacy laws.
Records may take up to 14 days to be received in the mail.
