Anniversary certificates are available for couples celebrating 50 years or more of marriage by 5 year increments (55th, 60th, 65th)
Birthday Request Form
Birthday certificates are available for individuals celebrating major age milestones: 80, 85, 90, 95, 100 and every year older than 100.
