Online COVID-19 Vaccination Reporting for Out of Province Records
Out-of-province doses will only be recorded if:
- A child is younger than five years old and has received one or more doses outside of Ontario, to ensure they have completed the appropriate number of doses for optimal protection
- You are aged five years and older and have received one or more doses outside of Ontario within the last six months, to ensure future doses (if needed) can be given at the appropriate interval
Niagara residents who have received a COVID-19 vaccination outside of Ontario or from a non-Ontario vaccine stock (such as Federal stock received from the Canadian Armed Forces, Federal Corrections or Global Affairs) may submit their COVID-19 vaccination records to have them documented in the provincial COVaxON vaccination database.
Before completing this form, carefully review the following:
- Do not submit records of COVID-19 vaccinations received in Ontario. This form is for COVID-19 vaccination records received outside of Ontario only. After assessment, valid vaccine records will be added to the provincial COVaxON vaccination database.
- If your vaccine is received outside of Ontario but in Canada, proof of vaccine must be in the form of an enhanced vaccine certificate with a QR code. Consult with the relevant provincial or territorial website for information on how to get an enhanced certificate.
- We are no longer able to accept handwritten proof of vaccination documents on their own. Handwritten proof of vaccination documents must be accompanied by a scannable QR code or an additional electronic receipt issued by the clinic/pharmacy/health authority. Please contact the clinic that provided your vaccination and request additional proof of vaccination.
- This form is not for travel documentation purposes
- If you are visiting the area for less than 16 weeks, we will not upload your information into the provincial COVaxON vaccination database. Your current form of proof of vaccination is sufficient.
