Medical Treatment Memorandum

You can submit your medical-related documentation by either:

Sick Leave

All employees are required to provide one of the following within required timelines for their employee group: (see chart below)

  • A suitable medical certificate, or
  • Treatment memorandum and functional abilities report

After the first 10 shifts of absence in a calendar year due to personal emergency leave, the reporting timelines as indicated above (for full-time employees) will apply.

The submission deadline for all employees is:

  • Within 24 hours after seeing a health care provider, and
  • Within 10 calendar days following the first sick day

Treatment Memorandum and Functional Ability Reports

Print and take your form to your health care provider to complete.

Workplace Injury Leave (WSIB)

All employees are required to provide suitable medical certificate or Treatment Memorandum and Functional Abilities Report within 24 hours of seeing a Health Care Provider as a result of a workplace injury.
